Overview
This brief guide will provide instructions on how to assign consent forms to respondents from your respondent hub. It will also cover how to send consent forms to respondents from your respondent hub and how to view signed consent forms.
Upload a Consent Form
If you haven't uploaded a consent form yet, please review this article with instructions for doing so. You won't be able to assign consent forms to a respondent until you've uploaded at least one form.
Assign Consent Forms to Respondents
Start by navigating to the "Respondent Hub" section.
- Click on "Respondents" then "Respondent Hub".
- Click the "Notify & Invite" button at the top left of the 'Respondent Hub' page.
- Click the "Assign Consent Forms" button.
Choose the consent form(s) and respondent(s) that you would like to send to under the 'No Forms Assigned' category.
You can also see which respondents you've already sent forms to or send again from the 'Already Signed' section.
Sending Tech Check Invitations with Respondents Consent Forms
You can now choose which respondent(s) to send the Tech Check invitation to, with the attached consent forms. To confirm you are sending the right consent forms to the right respondent, each respondent will have their email and assigned consent forms listed under their name.
Any session with consent forms assigned to the session will prompt a special bullet in the body of the default email, informing the respondent that they will be required to sign consent forms before joining a session to enter the interview.
Once you have reviewed all your information, click the 'Send' button in the bottom right-hand corner of the page.
Note: If there are any assigned consent forms to a session that a respondent does not complete before the interview process, they will still be required to sign consent forms right before trying to enter the session, in the case they are provided with the link to the session. Please refer back to the related articles on consent forms for more information on that process.
Consent Forms Assigned to Respondents
To check if a respondent has been assigned consent forms, view the form status column next to their name in the Respondent Hub. The assignment status will display as "Not Assigned", "Not Sent", "Not Signed", or "Signed".
You can also check if a respondent has been assigned a consent form by clicking the 'Add/Edit Respondents' button and confirming a green checkmark under the consent form column. Make sure a respondent is scheduled for a session to ensure the right consent forms are being sent to them. To learn more about scheduling respondents, visit the 'Respondent Hub' documentation.
Consent Form Safety Net
In order to guarantee every respondent has signed the form before entering a session, we recommend assigning consent forms to the interview(s).
Note: Your respondents who have been assigned this previously will not be required to fill out the form again if they are using the same email address that has captured consent.
